Our Commitment to Your Privacy
Soothe IV Med Spa is required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this notice describing our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

- Treatment: to provide, coordinate, or manage your care, including sharing information with your prescribing provider and dispensing pharmacy.
- Payment: to bill and collect payment for services, including verifying coverage or processing charges.
- Health Care Operations: for quality assessment, staff training, and business management.
- As Required by Law: including public health reporting, health oversight activities, and in response to a valid legal order.
- Other uses or disclosures not described here will only be made with your written authorization, which you may revoke at any time.
Your Rights
- Right to Access: you may request a copy of your health records in the form and format you request, if readily producible.
- Right to Amend: you may request that we correct information you believe is incorrect or incomplete.
- Right to an Accounting of Disclosures: you may request a list of certain disclosures we’ve made of your health information.
- Right to Request Restrictions: you may ask us to limit how we use or disclose your information; we are not required to agree except where the disclosure is to a health plan and you’ve paid out of pocket in full.
- Right to Confidential Communications: you may request that we contact you in a specific way or at a specific location.
- Right to a Paper Copy: you may request a paper copy of this notice at any time, even if you’ve agreed to receive it electronically.
- Right to Notice of a Breach: we will notify you if there is a breach of your unsecured health information.
Our Duties
We are required by law to maintain the privacy of your health information, provide you with this notice of our legal duties and privacy practices, and abide by the terms currently in effect. We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as information we receive in the future; the revised notice will be made available on our website and at our location.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
