Sildenafil vs. Tadalafil: How PDE5 Inhibitors Work

Sildenafil and tadalafil are not peptides either — they are small-molecule drugs in a class called PDE5 inhibitors, and they sit in the intimate-wellness portion of the program. They share a mechanism and differ substantially in one property: how long they last. That single difference drives most of the clinical decision between them.

The short answer

Sildenafil and tadalafil are phosphodiesterase type 5 inhibitors. Both work by blocking the enzyme that breaks down cyclic GMP, allowing cGMP to accumulate, smooth muscle to relax, and blood flow to increase in the relevant tissue. The clinically important difference is duration: sildenafil has a half-life of roughly four hours, tadalafil approximately seventeen and a half. Both are FDA-approved as tablets; orally disintegrating formulations are compounded.

The shared mechanism

Erectile function depends on a nitric oxide signaling cascade. Sexual stimulation triggers nitric oxide release in the relevant tissue, which raises levels of cyclic guanosine monophosphate — cGMP. Elevated cGMP relaxes the smooth muscle of the arteries supplying the tissue, increasing blood flow.

The enzyme phosphodiesterase type 5 degrades cGMP, which is how the response resolves. PDE5 inhibitors block that enzyme, so cGMP persists longer and the vasodilatory effect is sustained.

A consequence that follows directly from the mechanism, and that patients frequently misunderstand: these medications do not initiate the response. They amplify and prolong a cascade that sexual stimulation starts. Without that trigger, there is no nitric oxide release, no cGMP elevation, and nothing for the drug to preserve.

The half-life difference and what it means

Sildenafil has a half-life of approximately four hours. It is taken in anticipation of activity, typically within a defined window beforehand, and its effect is correspondingly time-bounded. Absorption is meaningfully slowed by a high-fat meal.

Tadalafil has a half-life of roughly seventeen and a half hours, which produces a substantially longer window of effect and supports a different usage pattern — including lower-dose daily regimens where the goal is continuous availability rather than timed dosing. Food has less effect on its absorption.

Neither profile is superior in the abstract. The choice depends on how a patient actually wants to use the medication, alongside their medical history and other medications — which is what the clinical evaluation is for.

Contraindications that are not negotiable

The most important is absolute: PDE5 inhibitors must not be combined with nitrate medications in any form — including nitroglycerin for chest pain, isosorbide preparations, and recreational nitrites sometimes called "poppers." Both act as vasodilators through overlapping pathways, and the combination can produce a severe, potentially life-threatening drop in blood pressure. This is not a caution to be managed; it is a contraindication.

Alpha-blockers, commonly prescribed for benign prostatic hyperplasia or hypertension, require careful timing and dose consideration for related reasons. Significant cardiovascular disease warrants evaluation before prescribing, in part because sexual activity itself carries cardiac demand.

Additional considerations include a history of non-arteritic anterior ischemic optic neuropathy, certain retinal conditions, severe hepatic or renal impairment, and specific drug interactions. What the compounded ODT form changes

Sildenafil and tadalafil are FDA-approved as conventional oral tablets. An orally disintegrating tablet — ODT — is a compounded formulation designed to dissolve in the mouth without water.

The reasons for preferring that form are practical: swallowing difficulty, convenience, discretion. The compounded ODT has not been through FDA pre-market review in that form, which is the relevant regulatory distinction and the reason the compounded-medication disclaimer applies here as it does elsewhere in the program.
